Dental Purity shall provide, whenever possible, "Expedited Servicing" on all DP-360's returned for service to our address 4511 NW 42nd St, Lincoln, NE 68524, and shall endeavor to complete servicing as quickly as possible but no later than nine (9) business days. Returned items for service must be in packaging approved by UPS or Fed Ex. The Customer is responsible for all shipping charges to and from the Service Center. Dental Purity is not responsible for damage if shipped in packaging that does not protect the unit from shipping damage.

Pure & Secure, LLC's Dental Purity Service Center will give a preliminary diagnosis and provide a work estimate. This work estimate will be communicated to the customer prior to doing any additional work and no work will be performed without approval by the Customer. Before sending any products into the service center, complete this form.

After servicing, unit will be shipped FedEx Ground, unless otherwise requested by ADMI. Dental Purity will maintain a servicing record and provide said record to ADMI upon request.
